For multiple Candidates in one family, please complete a separate form for each person.

If Candidate was NOT baptized at St. Mark's, we need a copy of their Baptismal Certificate.


The fee for St. Mark's Confirmation Program is $100 for the two-year program.  Please note check# below and drop off or mail to St. Mark's (Attn: Stephanie Oswald)

In addition to attending religion classes, I agree to enrich the faith of my child at home, including regularly attending Mass as a family.
